Whats the cause for Mold in homes?

I have a small 2 bedroom home that I rent. Since the day i moved in, there has been a moisture problem. And its in the middle of Winter “the dry season i thought”. All of my windows collect moisture and then rapidly grows mold in the windowsills and in the corners of some of the rooms by the floor boards, too. I keep the fan on in the bathroom 24/7. And dehumidifiers don’t help. Below both the front door and the back door I get a strip of frost sometimes ICE inside the house below the bottom of the doors. Help? Is it Serious?
The Windows have recently been replaced in the entire house! They are brand new! The Furnice isn’t kept hot, its at about 68 degrees, and as for the weather stripping both doors have it too! Someone suggested that the Whole house has a ventilation problem…. or the house may be too Sealed up… ????
